Facebow being dental equipment is used widely in prosthodontic dental treatments. It is closely related to a caliper and is used widely to record the relationship between the maxillary arch and spatial region or (to record the relationship between the maxilla concerning the transverse horizontal hinge axis of mandible), using some cranium or anatomic reference points and the relationship is then transferred to an articulator to prevent occlusal errors.  

Face bow dental consists of the following parts.

– A-frame that is usually in U shape, all the other components of the face bow are attached to this frame by multiple clamps. This U shape frame runs along the TMJ from one side of the face to the other side of the face. 

– Condylar rods, these are metallic balls on both sides of the U shape frame, at its free ends, to place over the TMJ. Or in some earpieces are attached to fit the external auditory meatus. 

– Bite forks, these are plates attached to the occlusal rims. A wax material is put on top of it, to record the occlusal orientation. 

– The locking frame, helps the bite fork to attach it to the U shape frame subsequently, recording the occlusal relation. 

– Third reference point, is used for facebow orientation. It gives an anatomical reference point on the face, that is used as a third reference point, in addition to the two reference points made by condylar rods.  

Facebow assembly and disassembly steps:

– Attach the nasion relator to the crossbar of the U shape frame, and tighten it with its screws. 

– Now attach the stem assembly. Make sure to attach the shorter length vertical post into the crossbar, and tighten it with its screws. 

– Now place the impression or wax on the bite fork, in the incisal area, and the molar area on both sides. The mark on the bite fork corresponds to the mid dental line. 

– Now put the bit fork inside the patient’s mouth, similar to taking an impression, but in this case the incisal edge of the upper centrals on the impression or wax. Cotton rolls can also be placed beneath the bite fork so that it is easy for the patient to bite and hold. 

– Now ask the patient to put the earpieces of the face bow into the ears. Adjust the nasion relator against the patient’s nasion. Meanwhile, slide the double clamp over the bite fork stem and tighten the big screw at the front. 

– Now adjust the face bow so that the orbital pointer is the same height as the point on the nose.

– Then tighten the clamps on the stem to support the facebow. This way the dental facebow instrument will capture the plane of occlusion, to the plane of reference of the patient. 

– Now loosen the screw of the nasion relator and then lose the large screw on the front. 

– Instruct the patient to retract the earplugs from the ears and open the mouth, now gently remove the bite fork, and then remove the facebow. 

– Now remove the screw on the crossbar to remove the bite fork assembly. The bite fork assembly is now ready for laboratory processing. 

Face bow transfer procedure to articulator: 

Once you have the bite fork assembly, 

– Set the incisal pen at zero.

– Add a mounting plate to the upper frame. 

– Then place the bite fork support to the mounting plate. 

– Now put the bite fork assembly to the mounting fixture and tighten the thumbscrew.

– Index the upper cast to the bite fork in the articulator. 

– Close the upper frame to see how plaster is needed, to make a connection between the cast model and mounting plate. 

– Add plaster to the upper mounting plate, as well as to the upper cast. Now swing the lower frame around until the incisal pin touches the mounting fixture and make a connection between the upper mounting plate and cast model. 

– This way you capture the plane of occlusion, relates to the plane of reference, connecting the cast to the upper plate. 

– Now open the fully adjustable dental articulator, remove the cast along with the upper mounting plate for finishing the cast with plaster.
